Standard Form 86 (SF 86)

The U.S. Government will do background investigations to assess the eligibility for applicants or employees to access to Federal facilities, automated systems, or classified information. All security clearance candidates must be thoroughly investigated and favorably adjudicated to hold a position as a Federal employee, contractor personnel or military personnel. These security clearance investigations are also a requirement for being issued a credential and access to classified information and secured work spaces. The Standard Form 86, “Questionnaire for National Security Positions” is intended specifically for use in requesting investigations for persons seeking to occupy positions designated as National Security “Sensitive.” The SF 86 is essentially the first step in your security clearance investigation process. Once completed and submitted, it becomes a permanent document in your record that is used for future investigations and your responses to this form may be compared with your responses to previous SF 86 questionnaires.
